(教育研究系)新規登録希望者 各位
【対象者】
今回、熊本大学で初めて放射線取扱者として登録する教職員・学生
※電子顕微鏡（加速電圧が1MV以上のものを除く。）のみの利用者は、放射線取扱者登録は必要ありません。（教育訓練・健康診断不要）

１）登録申請を行う
熊大ポータルの「放射線取扱者個人管理システム（PMSR）」により１０月１３日（木）までに申請する。
２）教育訓練を受ける
【講習Ｘ】 ※eラーニングにより実施いたします。
＜eラーニング開講期間＞
・１０月１７日（月）～１０月２８日（金） 時間：終日開講
・ビデオ視聴、テスト受験及び受講証明書の印刷を必ず行って下さい。
（上記全てを満たしていないと未受講となります。）
＜受講方法＞次のサイトにアクセスして受講して下さい。

【講習Ａ】
日時：１０月１９日（水） ８：５０～１６：３０
場所：アイソトープ総合施設6階講義室
＜持ち物＞
○教育訓練テキスト（紙媒体又はタブレットによる持ち込みどちらでも可）
○受講票
※風邪及び発熱等体調不良の方は、受講しないようにお願いします。また、受講当日は、マスク着用のうえお越しください。
３）健康診断を受診する
【血液検査および検診の実施日時・会場】
○１０月１３日（木） １２：１５ ～１２：４５（黒髪北地区保健センター）
※放射線取扱者の分類が、「放射線業務従事者」に限る。
↑この分類は関係資料の「放射線施設一覧」から確認すること。
※更新申請者、登録者で７月期の健康診断を受診しなかった者も受診すること。
【持ち物】
記入済み問診票（立入前）（職員用と学生用の各書式あり）
※７月期の健診を受診しなかった者は、問診票のほか、
放射線取扱者手帳（被ばく線量測定結果を貼付）も持参すること。
◆検査当日はマスクを着用し、受付整列時に前方との距離を１メートル以上空け、近距離での会話や大声の発生は控えてください。
◆発熱や風邪症状がある方は会場に来ないでください。

1. *Applying for Registration*
Please make sure that you apply for registration by SOSEKI before October 13th (Tue).

Date: October 19th (Wed) 8:50 – 16:30 Place: Radio Isotope Center 6th floor
*Please bring the lesson sheet and text.
*If you have fever or cold-like symptoms, don’t take a lesson. In addition please wear a face mask when you take a training.
3. *Medical Check*
Date and Place: October 13th (Thu) 12:15 – 12:45 @Health Care Center, Kurokami North Campus
<Important Items to Bring>
● A medical interview sheet
*Each form can be downloaded from “http://www.kri.kumamoto-u.ac.jp/notes”
Students who undergo the Medical Examination must hand in a medical interview sheet (【Before Entry】) at the venue.
*Those who forgot to attend a consultation for the July period can have a medical examination. Apart from the interview sheet (Periodic), please bring a radiation Worker’s Handbook (radiation exposure dosimeter result pasting).
<Other Notes>
Please wear a mask, keep a distance of about 1 meter from the front when lining up at the registration, and refrain from talking or shouting at close range.
If you have fever or cold-like symptoms, please do not come to the venue.
